In order to calculate change failure rates, we rely on at least one-years’ worth of historical data from our data warehouse, which comprises 160 client accounts’ operational IT data. By considering a fine-grained change category (see Section II B) and change closure code (successful, failed, issues, backed-out), we can compute the percentage of failed changes in each category.
